How to fill out an accident report correctly?

General recommendations

  1. The notice set consists of two self-copying copies of the form fastened together. You should fill out the front side of the “top” copy with a ballpoint pen on a hard surface, otherwise the text and marks in special fields will not be printed well.
  2. When registering an accident, blank notification forms from any driver can be used.
  3. The presence of blots, errors and typos in the notice leads to problems when registering an insurance event.
  4. If the second driver refuses to fill out the notice, fill out the document yourself. Be sure to indicate the make and model, state registration plate and color of the vehicle of the other participant in the accident. If there are witnesses, please include their details in the notice.

Procedure in case of an accident with several vehicles

  1. Fill out two sets of accident reports: one with the driver who was in front of you at the time of the accident, the other with the driver who was behind you at the time of the accident.
  2. If your vehicle was “pushed” onto a vehicle that was in front of you, indicate this circumstance in paragraph 15 of the notice. Indicate in each notice exactly who “pushed” your vehicle.

  1. The scene of the accident. Indicate the address of the accident site to the nearest house or kilometer of road. Example: Moscow region, Elektrostal, st. Pobeda, 12.
  2. Date of the accident. Indicate the date and approximate time of the accident. Example: 02/15/2015; 15:00.
  3. Number of damaged vehicles. Indicate the total number of vehicles damaged as a result of the accident. Example: 2.
  4. Number of wounded (persons who received bodily injuries) and deaths. Indicate the total number of people injured and killed as a result of the accident. If there are no casualties or fatalities, enter the number “0” in the appropriate boxes. Example: number of wounded 0, dead 0.
  5. Was the participants in the accident examined for intoxication? Indicate whether one or more participants in the accident were examined for alcohol intoxication. Place a check in the appropriate box.
  6. Material damage caused to other vehicles (except “A” and “B”) and other property. Indicate whether damage was caused to other vehicles other than the vehicles of the participants in the accident filling out this notice, and to any other property. Place marks in the appropriate boxes.
  7. Witnesses to an accident. Indicate the last names, first names, patronymics (if any) and the address of residence of witnesses to the accident. Example: Petrov Petr Petrovich, Moscow, st. Main, 15, apt. 13, Sidorov Igor Ivanovich, Moscow, st. Pyataya, 11, apt. 12. If there are no witnesses, write “no witnesses.” If the witnesses are passengers of your vehicle or the vehicle of another participant in the accident, underline their names to clarify this circumstance.
  8. Was the registration carried out by a traffic police officer? Indicate whether a traffic police officer participated in the registration of the accident. Place a check in the appropriate box. If the accident was registered by a traffic police officer, indicate his badge number. Example of a badge number: 063222 MIN.

Columns for separate filling

Each participant in the accident fills out his half of the notice (Vehicle “A” or “B”). The selected side of the notice does not in any way affect the procedure for determining the degree of guilt of the drivers.

  1. Vehicle make and model. Vehicle identification number (VIN). State registration plate of the vehicle. Vehicle registration certificate. Fill in the fields in accordance with the data specified in the Vehicle Registration Certificate. Examples: Make and model – VAZ 21083; VIN – XTA55441122111212; State reg. TS sign – X 945 RP 199; Vehicle registration certificate – 45MA 856745.
  2. Owner of the vehicle. Address. Indicate the last name, first name, patronymic (if available) and address of the owner of the vehicle or the name and address of the legal entity that owns the vehicle. Example: Ivanov Ivan Ivanovich, Moscow region, Elektrostal, st. Novaya, 17, apt. 7 or Zvezda LLC, Moscow region, Elektrostal, st. Sadovaya, 15.
  3. Vehicle driver. Date of Birth. Address. Telephone. Indicate the last name, first name, patronymic (if available), date of birth, address and telephone number of the driver who was driving the vehicle at the time of the accident. Example: Ivanov Ivan Ivanovich;
    10/13/1980; Moscow, st. Pushkina, 40, apt. 50, 8 926 743 00 23. Driver's license. Category. Date of issue. Indicate the series and number, category and date of issue of the driver’s license of the driver who was driving the vehicle at the time of the accident. Example: 5801 587432, “B”, 01/23/2012.
    A document granting the right to own, use and dispose of a vehicle. Indicate the name of the document on the basis of which the driver was operating the vehicle at the time of the accident. Example: power of attorney. Important! If at the time of the accident the vehicle was being driven by its owner, it should be indicated in the “PTS” or “Vehicle Registration Certificate” column. If the driver drove the vehicle based on the verbal permission of the owner of the vehicle, a dash should be placed in this column.
  4. Insurer. Indicate the name of your MTPL insurer in accordance with the data on the corner seal in the upper left corner of the policy. Example: Pobeda Insurance Company LLC.
    Insurance policy. Indicate the series and number of your MTPL policy, located in the upper right corner of the policy. Example: EEE 0198982892.
    Valid until. Indicate the expiration date of the contract indicated in the very top column of your MTPL policy. Example: 04/15/2017.
    The vehicle is insured against damage. Check the “yes” box if you have a valid CASCO policy. Otherwise, check the “no” box.
  5. Location of initial impact. Indicate with an arrow the location of the first impact with the vehicle of the second driver filling out this notice. You need to put a mark on either a large vehicle, a passenger vehicle, or a motorcycle, depending on the type of your vehicle.
  6. Nature and list of visible damaged parts and elements. List all injuries sustained as a result of this accident. In this case, hidden defects will be identified and described during inspection of the vehicle by experts. Example: the left headlight is broken, a crack on the front bumper, a dent and a scratch on the left fender. Important! The insurer may refuse to pay for the restoration of parts damaged in an accident, but not specified in the notice.
  7. Notes. In this column, the driver indicates all available comments, in particular, if necessary, supplements paragraph 16 “Circumstances of the accident.” The driver can also indicate that the second party to the accident provided inaccurate information in the notification. In addition, in this column you can enter information about the refusal of the second participant in the accident to sign the notice.
  8. Circumstances of the accident. Carefully read all the possible circumstances of the accident.
    Check only those boxes that apply to your case. If the listed circumstances do not correspond to your case, check the “Other” box (clause 23 for the driver of vehicle “A”, clause 24 for the driver of vehicle “B”). Indicate the number of marked cells. Enter the number corresponding to the total number of marked squares. Example: 3.

  1. Road accident diagram. Draw an accident diagram in accordance with the standard accident diagrams of the Russian Union of Auto Insurers. Attention! Be careful, do not make any mistakes or corrections! If there are contradictions in the information specified in clauses 16 and 17, the data from clause 16 takes precedence.

Column for separate filling

  1. Signatures of drivers certifying that there are no disagreements regarding clauses 14, 15, 16, 17. If there are no disagreements on the mentioned points of the notice, sign in the column corresponding to the part of the notice you filled in (“A” or “B”). In addition, you should sign under paragraph 15 in the column “Signature of the vehicle driver.” You only need to sign the part of the notice you filled out (“A” or “B”).

Attention! After disconnecting the forms, it is prohibited to change or supplement the information on the front side of the notice.

Reverse side (each participant in the accident fills out and signs his/her own copy)

Attention! It is not necessary to fill out the back side of your copy of the notification at the scene of the accident. This can be done later, for example, at home or at the insurer's office.

If there is not enough space on the back of the notice to provide the necessary information, write down additional information on a blank piece of paper. Be sure to write at the top of the sheet “Attachment to the “Notice of Road Accident”, and sign at the bottom with a transcript. In this case, you should put a mark in the “With attachment” box on the back of the notice.

  1. Vehicle. Indicate which side of the notice you filled out (“A” or “B”). Place a check in the appropriate box.
  2. Circumstances of the accident. Describe the circumstances of the accident in accordance with the notes in paragraph 16 and the accident diagram in paragraph 17. Example: 01/24/2016 I, Ivan Ivanovich Ivanov, driving a VAZ 21083 state vehicle. reg. sign A 574 RP 199, moving along the street. Mira towards st. Sadovaya in the right lane at a speed of 40 km/h.   3. The vehicle was under control. Choose an option depending on your situation. If the vehicle belongs to you, check the “vehicle owner” box. If you drove the vehicle on other grounds, check the box “another person authorized to drive the vehicle.”
  4. If more than two vehicles were involved in the accident, provide information about these vehicles. This column contains information about all vehicles involved in the accident, except for the vehicles of drivers who filled out this notice. In this column you should indicate the make and model of the vehicle; state reg. sign; FULL NAME. owner of the vehicle (name for legal entities); address of the vehicle owner and his telephone number; FULL NAME. driver of the vehicle (at the time of the accident); driver's address and telephone number; name of the insurer; series and number of the MTPL policy.   5. Damage to property other than the vehicle. Indicate the name of the damaged property and its owner. Example: Name – Road barrier. Who owns it – Unknown.
  6. Can the vehicle move under its own power? Please check the appropriate box depending on your situation. If the vehicle cannot move under its own power, also indicate where the vehicle is parked. Example: If “No”, then where is the vehicle now located - a guarded parking lot located at the address: Moscow, Yuzhny Prospekt, 34.
  7. Notes. In this column you can indicate any information about the accident that you consider significant. If there are no notes, the blank lines should be crossed out.

Lastly, indicate the date the notice was filled out, your last name and initials; sign.

Form and sample of filling out a notification of an accident

According to the law on compulsory motor liability insurance and the Traffic Rules, each vehicle is subject to compulsory motor third party liability insurance of its owners, as a result of which a compulsory motor liability insurance policy is issued.

This type of insurance allows you to cover the risks associated with the occurrence of road traffic accidents (hereinafter referred to as accidents). To receive insurance payments under compulsory motor liability insurance, a participant in a traffic accident must provide his insurance company with a notice of the traffic accident.

What is an accident notification?

A notification of an accident is a special act that confirms or denies your guilt in causing a traffic accident. As a rule, it is attached to the MTPL insurance policy in the amount of two pieces.

Notification of a traffic accident: deadlines for submission

This notice must be completed and delivered to the insurance company of the second party to the accident within three to five business days from the date of its occurrence, but not more than 15. If you wish to receive direct compensation for losses, then this form must be submitted to your insurance company. Conditions under which direct compensation for losses is possible:

As a result of the accident, only property was damaged;

There were two cars involved in the accident;

Step-by-step example of filling out an accident report

The front part of the form is filled out by each driver independently (each in relation to his own vehicle). The only exceptions are paragraph 1 8 of the General Information and paragraph 17 Road Accident Scheme, which are filled out by one of the drivers.

1 Collection of general information.

Filling out the front part of the notification of an accident begins with collecting general information about it:

number of damaged vehicles;

number of victims in the accident;

testimony regarding the state of intoxication (alcohol or drugs);

presence of material damage;

presence of witnesses to the incident;

if the accident was registered with the participation of traffic police officers, then the number of the police officer’s badge is indicated.

2 Data about vehicles and their owners.

Each driver fills in information regarding his vehicle, namely:

make, model and other data specified in the passport of the corresponding vehicle;

Full name and residential address of the owner of the vehicle.

3 Information about the driver of the vehicle who was driving it at the time of the accident, namely full name, date and year of birth, residential address, contact phone number, driver’s license number indicating the appropriate category and date of issue.

4 Data on insurance companies.

The name of the insurance company, details of the insurance policy (number and validity period) are indicated. If the car is insured under the CASCO program, then in the vehicle insured against damage column, the Yes checkbox is placed, but for compulsory motor liability insurance No.

5 Diagram of a traffic accident and a list of damage caused.

In the Place of initial impact column, the part of the vehicle where the first impact occurred is indicated (the area is indicated by an arrow).

In the column for descriptions of visible damage, it is necessary to indicate not only the damaged parts, but also the nature of their damage - a scratch, dent or crack.

To receive insurance payments for invisible (hidden) damage to a vehicle, it is necessary to additionally inspect it with the involvement of experts.

When displaying a diagram of a traffic accident, it is necessary to indicate the names of streets, directions of movement of cars, their position at the time of the collision, the presence of traffic lights and road markings and signs. It is recommended to display brake and skid marks whenever possible.

If inconsistencies are identified between the information, the information set out in paragraph 16 of this notice will be taken into account.

6 Circumstances of the traffic accident.

This item is intended to display the maneuver of your car. Here is a list of possible maneuvers; you need to check the corresponding box next to it. Filling out the column Indicate the number of marked cells is mandatory, since it allows you to avoid misunderstandings when assessing the circumstances of the accident.

7 Comments and approval of the specified data.

In the Comments column, you can provide information not specified in the Circumstances of the accident column. This item is endorsed by the signatures of the drivers, which confirms all the above data.

If the fact of an accident is registered without the participation of the traffic police, then it is necessary to additionally put signatures in paragraph 18 of this notification, which confirms the absence of disagreements between the parties to the traffic conflict.

It is important that both front pages of this Notice are signed. Please note that once the forms have been signed and separated, corrections and additions are not permitted.

It makes no difference which of the participants in the accident takes the original of this notice and who takes the tear-off (self-copying) copy. Both forms have the same legal force.

8 Filling out the back of the form.

The reverse side of the notice allows you to endorse additional information about the accident and vehicles. This side is filled out by each driver independently:

The appropriate vehicle is selected, followed by a brief description of the circumstances of the accident, indicating streets, directions of travel, etc.

How to file a notification of an accident according to the European protocol

Notification of an accident under the European protocol implies registration of the fact of the accident without the participation of the police, that is, paragraph 18 of the Notification is filled out. In addition, this form of recording allows you to confirm the right of the victim to receive payments under compulsory motor liability insurance. The maximum amount of insurance compensation under the Euro Protocol is 100,000 rubles (or 400,000 rubles, but only in cases where the circumstances of the incident are recorded using technical video monitoring equipment that records not only the image, but also the time of the event, as well as the coordinates of the vehicle (GLONASS/GPS system) ).

Accident Notification: Mandatory or Not in 2019

Notification of an accident is a mandatory document on the basis of which the amount of insurance compensation is determined. In addition to this, you can additionally provide a certificate of a traffic accident, which is issued by the traffic police.

It is not always possible to fill out this notification at the scene of the accident. The reasons may be different:

the second driver fled the scene of the accident;

refusal of one of the parties to fill out the Notice;

lack of forms and others.

If more than 2 vehicles are involved in a traffic accident and there are disagreements among drivers in assessing what happened, as well as if it is impossible for drivers to jointly fill out one form of notification of a traffic accident (for health reasons, in the event of the death of the driver, due to the failure of one of which from jointly filling out the form or for other reasons), each driver is allowed to fill out his own notification form indicating the reason for the impossibility of jointly filling out the notification of a traffic accident.

That is, this notification can be filled out unilaterally as soon as possible after the accident has occurred, but no more than 15 working days from the date of the accident. A notification of an accident under compulsory motor liability insurance is a document that contains a description of the circumstances, causes and details of the road accident. It is filled out by drivers, both in the presence of a traffic police inspector, and independently. This paper is needed so that the victim can receive compensation for damages under compulsory motor liability insurance from the insurance company.

What is a notification of an accident under compulsory motor liability insurance?

The notice is filled out at the scene of the accident - it contains information about which of the drivers is the culprit and which is the victim. Notification of an accident is submitted to the insurance company within 5 days from the date of the accident, and it is on its basis that a decision is made to pay insurance compensation to the injured party.

Road accident notification forms are issued by the insurance company each time a compulsory motor liability insurance policy is renewed, so it is advisable to always carry at least one copy of this document with you. You need to be especially careful when filling out the notice - blots and inaccuracies can lead to refusal to pay insurance compensation.

Good to know! In fact, a notification of an accident is the only document that is filled out when filing an accident yourself. One of its most common names is “Europrotocol”.

When notice is required

The need, as well as the specifics of drawing up and transferring a European protocol to the insurer, are determined by Federal Law No. 40 “On insurance of vehicle owners” and “Rules for compulsory insurance of vehicle owners.”

A notification of an accident must be filled out in all cases, regardless of whether the traffic police is involved in the registration or not - this will simplify the procedure for receiving payment from the insurance company. It is worth noting that in some cases, drivers can do without the help of a traffic inspector. In Art. 11.1 of Federal Law No. 40 provides the following circumstances under which an accident can be recorded without involving the traffic police, namely:

  • if the accident caused damage only to vehicles and (or) their trailers;
  • only when registering an accident that occurred due to a collision between two vehicles;
  • none of the drivers, passengers or pedestrians were injured as a result of the incident;
  • both drivers have a common view on the causes of the collision and agree on the question of which of them is to blame for the accident;
  • both drivers have a valid MTPL policy.

After filling out and signing this important document by both parties, both copies of the accident notice are sent to the insurance companies where each of the participants in the accident is insured within 5 days. The victim must attach to his copy a statement of direct compensation for damages.

In what cases is the participation of a traffic police officer mandatory?

It is not always possible to fill out a European protocol, which is why Federal Law No. 40 regulates the above cases in which you are allowed to record the circumstances of an accident yourself. Clause 2.6 of the Russian Traffic Regulations contains the following:

  • If the accident results in harm to life and health, then you cannot do without calling the traffic police.

Important! The presence of the slightest bruise or scratch on one of the participants in the accident is a reason to contact the traffic inspectors for proper registration of the accident. It often happens that a person is in a state of shock and claims that he is completely fine, and after a couple of hours he has to seek medical help. This may result in a refusal to pay insurance due to improper registration of a traffic accident.

Other good reasons for the involvement of a traffic inspector:

  • the participants cannot come to mutual agreement because they have different views on the causes, circumstances and outcome of the accident. This case is not always the basis for a patrol inspection. Most often, the traffic police officer gives recommendations regarding the collection of evidence to both drivers over the telephone and invites them to the department to clarify the details;
  • more than 2 vehicles are involved in a traffic accident;
  • in the accident, it was not the car that was damaged, but other property (building, pole, road sign, etc.);
  • the victim believes that more than 50 thousand rubles will be required to restore the vehicle;
  • one of the participants in the accident does not have valid MTPL insurance or owns a CASCO policy, the terms of which require an examination by traffic police officers.

Where to get forms

As already mentioned, 2 copies of road accident notification forms are issued when applying for an MTPL policy. It is advisable to always have them with you. What to do if the driver forgot to take them with him or does not have them at all, since he issued an insurance contract online?

  1. To obtain European protocol forms, you just need to go to your insurer’s office at any time - they will never refuse to issue an extra form.
  2. Insurance companies try to leave blank notice forms at gas stations and post offices.
  3. If the document was not available during the accident, and you need it right now, you can ask passing drivers for it.
  4. The notification form can be downloaded from the Internet and printed.

Filling out a form printed from the Internet is not very convenient and takes twice as long. The document issued by the insurer has a self-copying basis, so drawing up a European protocol on such a form is faster and less likely to make mistakes.

Rules for filling out a notification of an accident

Drivers fill out the European protocol form at the same time. Of course, this means that they enter the data one by one - each fills out their part of the notice. The document contains the following information: about insurance companies, the date and place of the accident, information about the insurance of participants, the circumstances of the accident and a description of the damage received. On the reverse side, drivers can further describe the details of the incident.

It is better to fill out the document with a ballpoint pen - the pencil is easy to erase, and gel ink smudges and does not print well on carbon paper. Filling out the front side

The first 6 points on the front side of the notice are information about the place and date of the accident, the number of injured people, damaged vehicles, etc. As a rule, no questions arise when filling out these and subsequent items. Please pay attention to filling out the following fields:

  • paragraph 7 – witnesses. This is an important aspect that greatly helps to receive an insurance payment as soon as possible, especially if the culprit fled the scene of the accident. If there are passengers or eyewitnesses, their data is needed - full name and contacts;
  • item 13 – indicate the location of the initial impact, marking it with an arrow in the figure;
  • clause 14 – nature and list of damaged parts and elements. At this point it is necessary to describe the damage received as concisely and at the same time briefly. It is also recommended to state that the driver does not exclude the possibility of hidden damage;
  • paragraph 16 – it is necessary to note under what circumstances the accident occurred (I was making a U-turn, leaving the parking lot, etc.)

At the bottom of the sheet, a diagram of the accident is depicted as clearly as possible and the signatures of both parties are placed under the information that each driver filled out, and in the middle of the sheet - in order to confirm the absence of disagreements.

Filling out the sheet on the back

On the back, each driver describes his vision of what is happening, notes whether the car can move under its own power, what other property was damaged, and indicates additional information about the insurance of other participants in the accident (if there are more than two of them).

At the bottom of the document, notes are filled out, the date, signature and name of the driver filling it out are put.

What to do next with the notification

According to the law, the notification completed by both parties must be submitted within 5 days after the incident. If the culprit does not do this, his insurance company will refuse to pay for the damage and will oblige him to pay for repairs to the victim’s car from his own pocket.

If the circumstances of the accident were recorded by the traffic police, then the culprit is not obliged to hand over the data.
